• 2008 Registration Form - filled out completely. (name must match birth certificate exactly)
• Original birth certificate (with raised seal)
• Medical Form - Form available on this site must be used
• Copy of complete 2007-2008 report card (with all 4 quarters)
• Signed - Parent's Code Of Conduct (by both parents/guardians)
• Signed - Rules And Regulations For Football And Cheerleading
• Photograph (cheerleaders only- players have picture taken when they receive their game jersey's)
• Photo release form
• Uniform Agreement
• Volunteer/Fund-raising Agreement
• Participant/Parent Waiver
Registration Fee:
1child = $175
2 children = $300
3 children = $450 max per family
(*Included in this price is: $25 raffle tickets)
*** Registration received after June 30th are subjected to a $25 late fee No Exceptions**
